how do i set this problem up using the logistic growth model?
Assume that the relative rate of spread of a certain disease is constant. Suppose also that in the course of any given year the number of cases of the disease is reduced by 20%. If there are 10,000 cases today, how many years will it take to reduce the number to 1000?
every year it reduces by 20%, ie it gets multiplied by 0.8, so
10,000 *(0.8)^y = 1000
0.8^y = 0.1
y*log 0.8 = log 0.1
y = log 0.1 / log 0.8
= 10.3188 yrs

First Time Growers Guide to Growing Pot That Teaches EVERYTHING - Ready, Set, Grow (Vol 2, Part 6)
How should I go about to find an a single growth rate to use if I have more than one growth rate?
I have a set of 12 different growth rate, which came from the population growth of the world between years. If I wanted to find a single growth rate to predict when the population would come to a specific number, which growth rate should I use? Do I take the average? But if I do, that will be TOTALLY different and the answer will be dramatically larger or smaller. What is the best bet?
Your question is not quite clear: where did you get the 12 growth rates from? Are they what it was in each of 12 consecutive years? Or is it from 12 different regions over the same time frame?
In the first case you should work out what the growth rate over the 12 years as a whole was and use that. To do that put each growth rate as a factor - 8% as 1.08 for example, and multiply them all together. Then take the 12th root.
In the second case you should use a weighted average (with weights according to population) to get the growth rate for the whole population.
